Homemade, it's just a piece of felt with cutouts. I saw him use it on a concert once.

I've used a wool beret (available very cheap on eBay) for the same purpose. (And can be very colorful, too.) Hangs on the the bell quite effectively. Never tried cutouts – don't seem necessary. Effect is sort of like a less muffled Softone mute. Adds fun (visual and audio) to any gig.
